 SOCIAL SERVICES
The hospice social worker is trained to assess patient and family needs and is able to coordinate community services for the patient and family - such a Meals On Wheels, transportation, and financial and legal resources. If the patient or family wants or needs a higher level of care such as assisted living, group home or nursing home, we can facilitate placement. The social worker has expertise in counseling and will provide support and guidance with issues surrounding the care of the patient and the needs of the family. The social worker also can explore alternatives for patient and family situations which are occurring in the present, have occurred in the past, and might occur in the future after the death of the loved one.

Social Service Support


Professionally trained, licensed social workers are available to patients and family members dealing with a life-limiting illness. The role of the social worker may include the following:

  • Discuss end of life issues with the patient and family, which may include Living Wills, DNR, Durable Power of Attorney, and Durable Power of Attorney for Health Care

  • Provide crisis intervention to resolve problems the patient and family may experience

  • Act as a liaison with community agencies, nursing homes, group homes, and assisted living facilities

  • Assist family and loved ones in the funeral planning process

  • Assist the patient and family with financial issues including Medicaid applications

  • Provide caring, compassionate, objective and non-judgemental advocacy for the patient and families
