 MISSION
The mission of Creekside Hospice, Inc. is to provide comprehensive, compassionate care for persons at the close of life so they may live as fully and comfortable as possible and to offer support to the caregivers and bereaved.

 GOAL
PICS
The primary goal of Hospice is improving the quality of life when the quantity of life is limited. This is achieved through palliative care and support services all designed to meet the patient and family's physical, psychological, and spiritual needs. Hospice care is provided by an interdisciplinary team utilizing a "holistic" approach to the care delivered to patients either in their own home or in a health care center.
 
Work in collaboration with other community agencies, Hospice strives to provide care to all who are eligible and need hospice care.

Creekside Hospice, Inc. is committed to providing the highest quality of hospice care possible in a cost effective and fiscally responsible manner.
 
Creekside Hospice, Inc. will be guided by the principles of RESPONSIVE CARE, TEAMWORK, LEADERSHIP, and STEWARDSHIP. Care to the patient and caregivers will enable those served to experience death with grace and dignity. The Governing Board, staff, and volunteers will work together to:
• Provide quality, compassionate hospice care and support.
• Educate the community about death and dying.
• Promote openness, trust, competence, and innovation.
• Acquire and wisely manage resources.
